located in the hospital entrance next to
Country Club Drive.
The home health program is designed to help
patients preserve their quality of life by
enjoying a healthier and more independent
lifestyle with a personalized and individualized
plan of care.
You may be entitled to home health care
services if:
(l) you are confined to your home
and cannot leave without the aid of
special equipment or
the assistance of another person and
(2) you are under the care of a
physician who certifies your skilled need
for home health care on
an intermittent basis. CRMC Home Health
accepts private
insurance, private pay, workers compensation and
Medicare
